Job function:

Provide direct personal care to individuals in their home, residential and or rehabilitation setting. Provide quality service delivery to persons, who are mentally ill, developmentally disabled, physically challenged and or seniors.

All staff must be able to demonstrate the ability to care for the needs of all our clients in the home and community. Each staff will receive an evaluation annually to ensure company level. Violation of the job description can result in severe disciplinary action or termination of employment.

Qualifications:

CLS Specialist shall not be less than 18 years of age and shall be able to complete required reports and follow written and oral instructions that are related to the care and supervision of residents. Be suitable to meet the physical, emotional, intellectual, and social needs of each resident.

1. Must have effective communication skills.
2. Follow all federal, state and traffic laws.
3. Observation, reporting, and documentation of a client’s status and the care or service provided
4. Maintenance of a clean, healthy, and safe environment.
5. Recognizing emergencies and knowledge of emergency procedures.
6. The physical, emotional, and developmental needs and ways to work with the populations served by the Agency including, the need for respect for the client and his or her privacy and property.

16. Appropriate and safe techniques in personal hygiene and grooming that include:
a. bed bath
b. sponge, tub, or shower bath
c. shampoo, sink, tub, or bed
d. nail and hair care
e. oral hygiene
f. toileting
g. safe transfer techniques and ambulation
h. normal range of motion and position
i. adequate nutrition and fluid intake
j. client rights and any tasks that the agency many choose to have the CLS specialist perform

18. Will report to Team Lead, and Human Resource.
19. Ensure quality and safe delivery of in- home services.
20. Participates in development and implementation of client plans of care if applicable.
21. No excessive usage of cell phone; this includes talking, texting & or surfing the internet

 22. Maintain exceptional attendance (absence & or tardiness more than 2 times per month that is unexcused is not acceptable). All tardiness/ absence must be reported to main office.

23. Must spend required hours with client per week as documented in IPOS.
24. Develop safety plans with clients (emergency evacuation: fire, tornado, utility failure, severe weather)
25. Increase the client’s community inclusion.
26. Assure that client receive an increase in social skills and resource acquisition. Byutilizing community resources for age appropriate activities to increase skill building; to improve social interactions with people in the community; interact with peers while participation in community activities.
27. Assure and encourage the client in the area of skill building in daily living skills by paying attention, self-management, self-loving, self- confidence/ self-esteem, self-control, patience, maintaining health/ safety, respectful to others and their personal space, age appropriate skill building techniques.

28. Support client in development of decision making and problem-solving skills.
29. Guide the client in developing decision making and communication skills. Aid the clientin the development of effective communication with individuals by listening effectively, communicating respectfully, talking in a calm tone, looking at the individual that is being communicated with.
30. Must attend monthly staff meetings and meetings with agency representative as requested.
31. Must document on progress note during each visit.
32. Must follow the Recipient Rights policy and procedures.
33. Must report all incidents and or accidents to Team Lead, and Human Resources within 24 hours.
34. Follow the IPOS or treatment plans for publicly funded clients.
35. Must not take client to your home or in the vicinity of your family or friends.
36. Perform any additional duties as assigned by supervisor.
37. Must complete all training; Refresher courses, test out, CPR and First Aid when needed.
38. Turn in progress notes every Monday.
39. Must have integrity and not falsify any information and or documents; oral or written.
